The monument was unveiled on 29. 11. 1951.
The letters on the plaque were renewed in 1996.
Dr. Černelča were shot by the Ustasha together with the nurse and medical sister in the village of Priseka (Prisjeka) (Prisjeka) near Stojdrag on the Croatian side.
